Absence Due to Vacation Form

This form must be compelted at least two (2) weeks prior to a student leaving for a vacation. Please ensure all information is accurate for proper recording and submission. The confirmation email must match the email on PowerSchool or this absence will be marked as Truancy.

**Misspellings or inaccurate information may result in the student being marked truant.**

**Students must complete all classwork and homework on the assigned due date. There will be no late submissions accepted for classwork and homework. Students will make up any missed tests, quizzes, and/or projects upon return as outlined by the teacher fo record.**
