Absence Due to Vacation Form

This form must be compelted at least two (2) weeks prior to a student leaving for a vacation. Please ensure all information is accurate for proper recording and submission. The confirmation email must match the email on PowerSchool or this absence will be marked as Truancy.

**Misspellings or inaccurate information may result in the student being marked truant.**

**Students must complete all classwork and homework on the assigned due date. There will be no late submissions accepted for classwork and homework. Students will make-up any missed tests, quizzes, and/or projects upon return as outlined by the teacher fo record.**
