Excused Absence Documentation Submission

This form is to be compelted for all parents/guardians submitting medical documentation for unexcused absences to be changed to excuse absences. All documentation must be submitted within 5 days of the absence for it to be considered. Please ensure all information is accurate for proper recording and submission. The confirmation email must match the parent/guaridan email on PowerSchool.
