SCHOLARSHIP APPLICATION FORM 2005-2006
Information for the applicant:

This Scholarship Application must be sent to the school no later than November 10, 2006 if you wish to compete for the scholarships.

There is only one administration of the test.

Scholarships are based on academic excellence on this test.

To be eligible for the renewal of the scholarship, the student must maintain honors along with a good discipline record and involvement in the school.

There is a $30.00 application fee which must accompany the application. Please make checks payable to "Father Judge High School."

You may also apply for other financial aid. When you come to the Open House, be sure to ask about the Tuition Assistance Form, which will be available at the end of December.

Bring TWO number 2 pencils to the exam.
